Maintenance Log For Palm

Somewhere I surfed across a downloadable maintenance log for the AV. It was designed for the Palm OS and looked pretty neat. Unfortunately, I have been unable to find it again, and am starting to wonder if I dreamed it up. Anyone else seen anything like that?
